Bleach gets it off my countertops and tilework.
I'd assume you'd hafta wash it thoroughly in regular bleach (if white) or bleach alternatives (if not white). For hygenic purposes, I'd suggest soaking the entire garment in the bleach of your choice for at least a couple hours, then washing it rigorously, and putting the load on for another rinse cycle before drying. Even after drying, I'd also hang it out in a well-ventilated area with some sunshine.
If not, try drycleaning, but point it out to the cleaners, and go to a reputable outlet.
Or, you should probably throw it away if the mildew is widespread. You can always replace clothes, but don't keep it if it is in bad shape -- it could keep the mildew in your drawers, closet, and your air vents. (Psst.. it's Fall - a good time for sales).
