As regards your GP the term doctor is just a courtesy title, however GPs who have a doctorate i.e. a Phd are really a doctor. It all comes down to whether you are using it fraudulently. It would be, as some people do,
reasonable to use the term Dr your-name with quotation marks around the Dr. It is interesting to note what the DVLA say about this when changing your name on a D1 form:
Use the same processes to change your title (eg Ms to Dr). You don’t need to send evidence unless it’s a ‘hereditary title’.

any one who claims to be a doctor and isn't has some deep psychological problems. It is illegal in the sense of acting as a doctor IE treating people, the same as impersonating a police officer.
If on the other hand you lie to get a girl/boy then you are obviously very uncomfortable with who you are and what you really do. (time for some self fact finding)

My Phd makes me a Doctor, but not a Medical Doctor.
If you are impersonating a Medical Doctor, and give any Medical advice, you can be prosecuted for practicing medicine without a license.
What is so hard about being honest?

If you actually are a doctor, whether you are a physician or someone who has a doctorate degree, then calling yourself a doctor is no problem. You just can't pretend to be something that you are not.

No. It is only illegal to practice medicine without the appropriate qualification. You can call yourself whatever you like, provided you are not attempting to defraud anyone.

If you are not certified, of course it is illegal to call yourself a doctor, that has serious legal implications.

You can call yourself whatever you want.
If you practice medicine and you are not an MD, you'll go to jail for sure. Then you will get sued for malpractice.
