The outer boot is always the closest to the tire. The inner is closest to the rack and pinion, or the rear differential. If you are changing the boots only, be very careful. You must clean out the contamination inside the joint very very good. Any dirt left inside the joint will tear up the CV Joint itself. Always use the lube recomended for the CV Joint only. You need to remove the shaft from the truck to do this properly. If in doubt, take it to someone who knows what they are doing.

First things first, you say it makes the vibration when in gear or out of gear. Do you mean that the car is driving and you put the car the car in neutral? If so then its possible that there is a problem with drive shaft CV joint. If however the noise is there when the car is stationary and you rev the engine, the problem is not with the drive shaft CV joint. Lay under the front of the car, look up at the drive shafts. Use your hand to feel the CV joint gators for rips or tears. Normally if the gator has split you should feel grease splattered all over the area around the CV joint. To replace the gator you will need to remove the drive shaft from the car, first jack up the front of the car. Remove the wheel, then split the lower tie end rod. The remove the drive shaft retaining nut (In the middle of the brake disc hub under the dust cap. Next you will need to pull the hub with the brake still attached out towards you as far as you can, you may need to tap the drive shaft with a hammer to slide it out from the wheel hub. Then twist the drive shaft away from the wheel hub so it clears the wheel hub, you will need to man handle the drive shaft to get it clear for the hub and probably you will need to man handle the hub. Once you have the drive shaft clear of the hub you will need to put a container under the gearbox side of the hub to collect the oil from the gear box. Next grip the drive shaft with both hands and pull it towards you, the drive shaft is normally held in place with a metal clip so give it a hard pull. Once the drive shaft is clear you will be able to strip, clean and re-grease the cv joint and replace the Gator. When you have the CV joint clean inspect it to make sure there is no damage to either the balls or the races. If there is then you are better of replacing the complete drive shaft with a service exchange unit, try ebay. Ja.

Buy a Chilton or Haynes manual, the inner boots are the ones closest to the front differential, the oters are the ones closest to the wheel. Its usually best to replace the axle as an assembly.

Get you a repair manual at a local parts store for procedures and the one closes to the tire is the ouside boot and the one closes to the transfer cases is the inner.

Get a Chilton's Guide for your specific make and model. They are kinda expensive, but very detailed. you may see if there is one for sale on ebay.
