A bird coming up to the house (to take a soul away--called a "psychopomp"--Greek for soul guide.)
Seeing the full moon through the branches of an apple tree.
Seeing a "double"--a person's soul projected from somewhere else to warn you of that person's death.
I've read a few places that if you go to a graveyard outside a church on Halloween, you'll see a procession of the people who will die that year go by at midnight (I guess it's their soul doubles.)

Considering that virtually everything is considered a death omen by someone, and everyone eventually dies, just by the law of averages most deaths are going to be preceeded by something someone would consider an omen.

I personally don't believe any of them, but have heard of the following:
1. Seeing a raven perched outside your window.
2. Having a dream about someone having a baby.
3. Hearing a owl hoot in the daylight.

The only death omen I have heard of is if a bird flies into your window and it dies. However, I dreamt that 3 cloaked men in black came for my mom and she did die 3 weeks later.But she had been ill and confined to her bed.

Death Marks -Big Black /puriplish marks on the body usually arms and legs that appear for no apparent reason.
2. Bird flying into a closed with or door twice ,sounds like a knock: then dies right then and there.

An owl hooting during the day, a black cat or dog, ravens. There's also one that only pretains to mining in the 18-early 1900s. When a miner's work clothes fell off their hook, he would fall down a mine shaft that day.

deff the Black bird in a building.
My friend use to work in a gas station at night. i was on the phone with her when she said a black bird flew and was standing on one of the shelves. I thought it was odd since birds dont fly around at night. about 2 days later.. i was on the phone with her.. when it happened again.. another bird flew in. It wasn't till the next day i remember the story .. if you see a black bird fly into a building its death.. a white bird is marriage. about 1 week later.. it happened for the 3rd time. the next day her step dad past away in a motorcycle accident

As a nurse I know that when I hear the 'death rattle' and the patient seems very far away spirit-wise, the time is close.
