If she has a blocked duct, believe it or not the best thing is to continue to nurse. Same goes for sore nipples. They have to "toughen" up and that won't happen if she stops nursing. Get her some Lanolin cream like Lansinoh for her nipples. It will soothe the friction and dryness. And if she has a fever, she can still breast feed. I was worried about nursing while I was feverish. I called the Le Leche League and they gave me a lot of great advice.

Go find a pharmacy that is open ASAP and buy Lansinoh, or another 100% Lanolin creme, for your wife. That really works for sore/cracked nipples - from experience! It's much better than other types of cream they might give you because this one you can leave on when breastfeeding and not have to worry about scrubbing clean your already fragile breasts.
Also, I agree with the other person who said that if she has blocked milk ducts, the best way to 'unblock' them is to continue nursing - as painful as that is! A LITTLE drink of wine or beer (and beer is suppose to help with supply anyways), a warm shower, some Lanolin cream, soft music, and then start nursing... and she'll likely need all of that if it is as painful as when I first started. I went and saw several different nurses to see if I had the positioning incorrect or if my daughter's latch-on was wrong as they SAID that was likely the problem, but no. We were doing well, we just had to push through that pain. Really support your wife right now as there is nothing like the shooting pain she is probably experiencing. My husband couldnt believe it, I made it through child birth without needing pain killers and I CRIED while breastfeeding the first 2 wks. It passes, really it does, just help your wife through this hard time. It's completely worth it, promise!
Here are some tips for cracked nipples if that is a problem - try to express a little milk and rub that on after a feeding, and if she can expose them to sunlight that is suppose to help them heal up faster too. Hopefully you dont live near me, because the sun has already disappeared for the winter, likely not to reappear until Spring!

If she is having nipple pain, a temporary relief until she can get to a lactation counselor is to put baby Orajel on her nipple. It will numb them enough to nurse. But, she needs to get help soon to make sure her breast is OK and that the soreness doesn't get worse.

Another idea are these things called 'lilypads'. They were awesome when I was nursing. They're plastic/silicone nursing pads at motherhood maternity. They don't rub, they stay in place, and hold quite a bit, although she will need to empty them quite often. I used to look like I was nursing a wolverine when I nursed my son. These really gave me a much needed reprieve.
