The official rule on home runs is:
"6.09 The batter becomes a runner when --...(d) A fair ball passes over a fence or into the stands at a distance from home base of 250 feet or more. Such hit entitles the batter to a home run when he shall have touched all bases legally."
So, it's only a homerun when the batter/runner has touched all the bases, not when he hits it over the fence.
As noted, there are a couple instances of players being unable to continue around the bases (having injured themselves swinging or running) and a pinch runner being allowed at the umpire's discretion.

Yes, the baserunner has to touch all the bases. One such instance is Game 5 of the 1999 NLCS between the Braves and Mets. Robin Ventura hit a game-winning grand slam, but he never advanced past first base because his teammates mobbed him. The official score of the game was 4-3 instead of 7-3.

ya, if you falls down and can't get up you are still credited for that run. but you have to get a pinch runner to replace him. That's what happened one time during a red sox game. One of the hitters for the red sox hit a homerun and the player (Gabe Kapler) rounding third just fell and everyone was laughing cuz they though he just tripped but it came out that he blew his achilles or something.

yes a pinch runner can come in and finish the trot
this happened a couple years ago when someone hit a homerun with Gabe Kapler on base and when Gabe was rounding second he tore his achilles tendon... the guy who hit the homer had to wait behind him on second base until they got Gabe off the field and a pinch runner came in and completed the trip.. remember, if you pass the guy in front of you while rounding the bases - hes out

Yes and if he gets hurt while running the bases on a home run a
substitute finishes running the bases to make it official.

a homerun is just another base hit, but that baseball couldn't pickup by defender. So, all base must be touched and can run slowly since no one is gonna out him.
