I would agree that both Se7en and Silence of the lambs are good options, in addition I saw a film called Hostel last weekend and would recommend it to you. Other films with a twist are The sixth sense, The skeleton key, angel hart, psycho (the original), the usual suspects, I could go on, but have to go to work now!!

Strange, I watched Saw and Saw 2 the other night and was thinking about posting a question like this myself. I am always on the look-out for films that make you think, films that make you wonder.
Neither Silence of the Lambs or Se7en will satisfy you - these films, while good, do not provide the total shock, twisty ending that you require.
As others have mentioned, Fight Club, and The Usual Suspects end in a way you would not expect, so maybe you should check those films out.
But, personally, I recommend:
The Score (starring Robert DeNiro and Edward Norton) - a heist movie on par, if not better than Ocean's Eleven (itself a good film if you haven't already seen it). The twist at the end, while a little predictable, still seems to surprise.
But I ultimately recommend The Jacket (starring Adrien Brody and Keira Knightley). It is a bit of a mind-f*ck, but a totally amazing film. It not only has a shock ending, but the whole film keeps you guessing.

I'm being serious, the official website is so screwed up - one glimpse of it and you want to watch the film.

Se7en is the best twist, ive watched a few times and it still makes me think 'oh my god'
Saw 3 is coming out soon, hopefully it will just as good as the first two if not better, they were great films
Hostel is good to, very gory and makes you think if it goes on in real life
Gothika is good has a twist that wouldn't think of

You absolutely have to watch "Dragonfly" It is one of the most awesome movies that you will NOT guess the ending until the very end. If you want a book like that I just finish "Cell" by Stephen King. It's about a signal sent through cell phones that change the people who are on a cell. The ending is totally 'Hitchcock'.

Silence of the Lambs is good... Anthony Hopkins is very creepy and scary at that film... he will scare the hell out of you.. it actually won an academy award for best picture..*i think... Hide and Seek??? its twisted too... Hostel is just plain bloody... it doesn't have twists or a good plot... it is disturbingly entertaining though...

There is a film i watched a couple of years ago TWICE lol and i still don`t get it , weird ending , the film was Mulholland Drive , a Stanley Kubrick one . Maybe you could watch that and u could explain to me the ending :)

Those are good suggestions. Also, try Hostel, The Machinist (with Christian Bale), Stay (with Naomi Watts) just a few more for you to see. Also some one else mentioned The Others (with Nicole Kidman) it is a must see!!!!!
