In India Form 60 is applicable if:

If you do not possess either PAN or a GIR (General Index Register) number but have transacted in cash for any of the activities listed as clauses (a) to (h) of Rule114B, namely -
a. Sale and purchase of immovable property valued at Rs 5 lakh or more
b. Sale and purchase of motor vehicles (other than two wheelers)
c. Time deposits exceeding Rs 50,000 with banks or banking institutions
d. Deposits exceeding Rs 50,000 in post office saving banks
e. Contracts for sale or purchase of securities exceeding Rs 10 lakh
f. Opening an account (other than time deposit) with a bank or banking institution
g. Application for the installation of a telephone or cell phone connection
h. Payments to hotels and restaurants in excess of Rs 25,000 at one time.

There is no general 'Form 60.' Not even on the IRS website or anything. The Federal Energy and Regulatory Commission has one, but its an Annual Report for Service Companies. That doesnt apply to you, nor should it have anything to do with you.

More than likely its an internal document specific to that company, unless they are running a scam, or were trying to blow you off. Just ask them for the form. Why didnt you do that anyway? Please make sure this is a legitimate credit card company. If you have to, use the better business bureau to check.

It is given in lieu of Income Tax (IT) PAN no. If you are nor filing IT returns you need to give Form - 60, which is a declaration, for new Credit Card or opening an Bank A/C. in India.

InIndia if you do not have a PAN number, you are required to fill form 60.

If you don't have salary certificate you have to fill form 60...if you want credit card do as per their request.

If you have PAN card you shouldn't bother. Ignore it. If you don't have you have to fill, they will provide you the form 60.
