It's derived from the French quinze, trente, and quarante, which represented the sections of a clock. 45 was simplified to 40 to make a euphonious sound between the four (love, 15, 30, 40).

Because the English made up the modern rules to the game, and they're all a bit dotty.

It's to do with the hands of a clock....but somewhere they lost their timing...
