It can mean any of those and it really depends on the context. Sometimes you can even say "we hooked up after the show" and it could just mean met each other/ got together or you could say "we hooked up that night and now I know he's the man I want to marry" and that might mean something different. Depends how perverted you are i guess...

To me it's when you meet someone somewhere, and then stay together for a while. Sometimes just friends meeting at the Pub, sometimes it's a guy making a move on a girl at a party and they spend the night together, whether it be sexually or otherwise. Hooking up = meeting up.
