With guys it means worked out or muscular. I wouldn't use it the same way to describe a woman though. If I heard someone describe a woman as "buff," I'd assume they meant she was good-looking, but not muscular.

More like "he's so buff." Means muscular and toned.

They shouldn't say that about a girl. It means like for a guy who works out a lot and has big muscles.
