Kernberg has a theory of personality disorders that range on a continuum from borderline to narcissistic to anti-social, in his opinion each being harder to treat than the previous one. Antisocial personality disorder in it's severe form are what we typically call psychopaths.
The prognosis for all personality disorders is fair-to-poor.
However, the worst psychological disorder, (which is highly subject to ones own opinion), is probably severe brain damage, as coded in the DSM IV. If brain damage is severe enough a person would be non-functioning and would be destined to laying in bed in an institution or long term care facility. There are a number of ways one can acquire brain damage. Some people are born with it.
Both personality disorders and brain damage are considered to be Axis 2 diagnosis'
I'm assuming your asking the question about Axis 1 diagnosis'.
One of the most common "psychological disorders" is considered to be a simple phobia, which is fear of public speaking. People get this from their socialization--- their experiences growing up. Plus fear of critisism is sort of a natural fear and may even be ingrained in societies.
One of the most common disorders a clinician sees is situational depression.
Schizophrenia can be a psychological disorder that could be considered to be one of the worst. Bipolar with rapid cycling, ranks right up there.
How do you get them?
Well... a person is probably factor loaded to get one of them based on genetics and whether or not it runs in the family. That, and usually a highly stressful event in ones life may trigger the onset of either of these conditions.

I'd think any type with dementia (such as Alzheimer's) would be the worst. I don't know enough about it to say how to get it for sure. The simplest one to get? Probably anxiety, and you can get that in different ways, autoimmune and allergy issues or wrong diet and activities and environment or everyone's easiest way of getting it: just not handling problems right away or the best they can.

Based on what I've read and heard - narcissism has to be near the top. One is born with it, though it can be harnessed at an early age, and it appears incurable, and the person who has it, though is relatively happy, is an expert of driving the more sensitive around them to suicide, through their own self-importance and ability to diminish others. This may not sound like much, but look up narcissism in the Internet - scary stuff!

Cytobionosis. While this disorder is not entirely psychological, it may cause chemicals to form in the hippocampus region of the brain, where they have a high chance of reacting with tissue and um.....exploding....
This condition is obviously deadly, but thankfully,
very, very, rare!
It CAN be treated if caught early and properly diagnosed.
Symptoms include, but not limited to:
weak motor-neurons, blackouts, "broken neck"
feeling and a disrupted biological clock (most common)

But scorching sauce or a few kind of liquid you do not like to your lip whilst you move to chew it you discover the scorching sauce (or what ever you decide upon to place on there). Then you will not desire to chew it hold doing this till you do not even feel of doing it any further. This additionally works for palms or finger nails. Hope it is helping ;)

Anyone that suffers from mental illness will probably say their condition is the "worst", because they have to live through it. Paranoid Schizophrenia is pretty brutal...it caused my great uncle to try and murder his children with a knife, and it has destroyed his life to where he is now homeless and eating garbage out of dumpsters. Personally, I feel that psychopathy is one of the "worst" mental illnesses out there, because a psychopath is incapable of feeling emotions...serial killers are often diagnosed psychopathic. And mental illness is not contagious...sometimes it is triggered by great trauma...but usually it is inherited through genetics, so if you have a number of family members suffering from mental illness, there is a chance you may have it.

I'm bi-polar. I think I have had it for a few years. I also have bulimia and have had anaorexia not that long ago.
