if you have a pic of the cat you can get a gold necklace made with it and then engrave the back with the dates, if not, try a vintage shop for the necklace then engrave the back...that is really sweet of you!

If you don't have your own jeweler then go to a jewelry store and explain what you are looking for and have them order one for you before they engrave it so you can have a look at it and approve it. Of course you should discuss the price and remember the chain usually does not come with the pendant. When you approve then tell the jeweler what you want engraved on the pendant. Where you want the words placed, the size of the letters, the style, etc. It is always wise to stick with one jeweler, of course you have to be happy with their work.

Any jewelry store should have a selection of cat charms which could be engraved and hung from a chain.

At some walmart's and other pet stores like Pet smart they have a pet tag ingraving machine. Maybe you could do it on one of those?
