There is also a book called "Codes of Ethics in Counseling and Psychotherapy"
by Elizabeth Welfel that will cover all your questions

in addition to the exceptions listed in the previous answer...being court ordered by a judge to release information is another exception.

and may or may not differ depending on the type of angecy you are working in. Working in a federally funded alcohol treatment center for example has slightly different confidentiality agreements due to stricter federal guidelines.

All facilities release some confidential information to their payer sources (insurances, medicaid) which is typically covered in the intake paperwork. However, once the information is transmitted to the payer confidentiality of that information cannot be guaranteed.
