TV is definately off, it's rude to watch and eat. Family meal time is for being with your family so you can talk and spend time together. If you're eating alone, do what you want.
Dog, doesn't matter as long as "Fido" isn't begging at the table. Mine can lay on the floor under the table unless they beg, then they have to leave the room. They know this and if they screw up, all I say is "OUT!" and they leave the room.

If you're alone, tv on is ok. If you have a bunch of people/family there, probably tv off. Dog is ok inside if he doesn't beg at the table. Otherwise outside.
BTW, my grandfather used to keep the tv on during dinner and shout at the tv every time the President came on.
