go to the post office get a form, fill it out send a copy of ur id and birth cert. with it mail it all together and 6 wks u got the card good luck

You will have to apply for a duplicate with the Social Security Office. Report right away that you have lost your Social Security card. You will need other forms of Identification.

Go to the social security administration office and they have a form you can fill out. You will need some kind of picture ID, and your birth certificate. If you don't have a state issued ID, then take your school ID, or something that connects you to your picture. Also, you may want to take a pay stub with your ssn on it. anything that you can provide that shows that you are who you say you are.

all you have to do is go down to your local social security building maybe it is downtown or you can look it up in the phone book just go down there and request a new ss# card and you will have to fill out a application and they will give you a piece of paper kinda like a receipt then in a couple of days they will send you one in the mail

Go to your local library and ask for a form if you are not in the state you were born in. The form will have questions that want to know your full name, what hospital you were born in, your mother's maiden name, fathers name, and other random personal questions. It is illegal to copy you birth certificate in color. If you have it send a black and white copy. It also beats sitting in the SS office all day.
