It means you're very cool/cold. Depending on the context, it could either mean you're heartless, unemotional and cold or else it could mean that you're calm, cool and collected. I think someone used the word 'unflappable'. I've heard it used both ways (but not about me - I'm pretty flappable).

If you're a cool character, so cool that butter (which melts easily) would not melt in your mouth, then someone might use this expression about you. Cool in this sense is not like teenagers would use the word, but cool as in composed and unflappable.

It means that you are so cold that butter wouldn't melt in your mouth. Its an insullt. It means you have no feelings and no compassion. Cold. Like a refrigerator.
