A word of advice, get a lawyer. Any money you save now by doing it yourself will be pennies compared to what it costs you in the long run by not having a lawyer do it for you.

Well here it would be San Diego Superior Court but your county/city will have a different name. All the forms should be there on their site.

if not call up some lawyers and ask them if there is a place you can go with out having to get it from them

you can go to offies maxx an get them for 39 bucks any one i tryed online did not work so i went there an got them
