You can make computer to computer calls using only a software program, a headset, and your broadband connection. It's called a softphone. If you want to call regular phones, or have a number that regular phones can call into, you'll want to get an adapter and sign up for a voip calling plan.
The adapter changes your voice into a form that can travel over IP lines. When I signed up for my voip.com account, they gave me the adapter for free. I pay $19.95/mo for unlimited calling in the US, Canada and Puerto Rico and also get international long distance for really low rates (like 1.9 cents a minute to Austrialia!). They also give me all the premium features like Caller ID, voicemail, call waiting, etc... as part of my calling package.

BTW, if you subscribe to an internet provider that does not require the use of a telephone line (non-DSL), you could eliminate your phone line and conduct you calls via the net, to non VoIP phones as well -- i do recall emergency services - such as the ability to dial 9-1-1, being a point of concern.
