You'll never know if he still feels the same way about you unless you ask him. Be honest with him, tell him how you feel and apologize if you hurt him. Ask if he'll give you a second chance now that you've realized how much he means to you and that you were wrong. Everyone deserves a second chance and hopefully he will give you one.

Well, just be honest with him. Chances are that if he is not dating anyone else then he has been pining away after you still... or would definitely want to still be with you. Just tell him you made a mistake and tell him why you made the mistake. Maybe it was out of insecurity, maybe you didn't feel that you deserved someone as good as him... but you really have feelings for him and you need to express those.
Whatever the answer, you will at least have closure about it.
