Give all the information that the application asks for; there is a place for your maiden name on it. There is also a place for both of your parents names.
Your on file birth certificate (not the one you have) will have your name change on it as well; this is part of what you pay to get a marriage license. This license should be on file in the state where you were born. Contact them for a new updated copy. You may have a small fee to pay for the copy.

No, a marriage certificate (I suppose you mean marriage license) has nothing to do with getting a passport. Go to the passport office in your city and apply for one, or, go your local post-office they have passport applications and can help you.

no, u think u need to be married to get a passport, just go to the post office get a picture taken, wait a few months and then its delivered at ur door-im 14, have traveled the world, i know teeeeeruuuuuuusssssstttt mwaz

You need all of those documents, including your marriage certificate because it states your present legal name. Don't forget your Social Security Card. Your SSN card should also reflect your name change so stop by your Social Security office first.

Not if you were born in the USA and/or are a citizen (which of course you are a citizen if you are born USA). You just have to have your birth certificate (or othe document that proves citizenship) to verify that you are a citizen.
