Down here refers to being depressed or unfortunate.
Kicking him is the opposite of helping him.
The analogy relates to a man lying down, and instead of helping him, you just walk over and kick him, to keep him down.

It means that a person is already suffering, and then someone or something else comes along and makes them suffer even more.
