he never dresses up for halloween (and he's an identical twin, if that helps at all) but we're going to a party and he wants a good costume. all i can think of is siamese twins, or that t-shirt that says "i'm the evil twin". i need some inspiration.

If his brother is going also they could be Dr. Jekell & Mr. Hyde... Or a couple of years ago I had some friends that dressed as Pimps, it was great! They were the hit of the party!
