I can see Andy Cole being the answer but, if it is then that's not entirely right as when he won promotion in 1993 with Newcastle it was still the 1st Division.
If he is not the right answer then I will be interested to see who it is as Teddy came up through the play offs with West Ham.
Maybe Gary Pallister/Paul Ince. Must be a Man Utd old boy from 1999, who then went elsewhere and got promoted.

nearest guess is Ryan Giggs - won the FA Cup and Premiership on numerous occasions,Champs league in 99 and League Cup last season
