No. It has never happened. However,it could have happened at the start of the 80's. Philadelphia played in all four championships consecutively and had that opportunity. The Flyers played in the 1980 Stanley Cup final and lost to the Islanders in six games. The 76ers played in the 1980 NBA final and lost to the Lakers in six. The Phillies won the World Series in 1980. And the Eagles played in the Super Bowl for the 1980-81 season and lost to the Raiders.

No. The closest any city has come in recent memory is Boston in 2007. The Red Sox won the World Series, the Pats went 19-1 and lost in the SB, the Bruins lost to the Canadiens in the playoffs and the Celtics won the NBA Championship.

I'm going to say "no". It's near-impossible because not all cities have 4 sports, and if so the teams might not even be traditionally good.
