Be careful! Rusty metal objects are often found outdoors, where Clostridium tetani resides. This bacteria can be introduced to the body by puncture wound, so a 'rusty nail' or such may bring it into the body. Clostridium tetani, a bacterium, releases a toxin that causes tetanus. Tetanus can kill you, but most people are immunized against tetanus and receive booster shots every ten years or so.
It is important to note that the rust is not actually an important factor. It merely indicates that some metal object has been outside and exposed to feces, etc., which carry 'tetanus'. If you were to put a nail into a sterile environment and then cause it to rust, you could not get tetanus from that nail.

Common "rust" is nothing more than iron oxide. Remarkably low toxicity and related deleterious effects to humans.
Yes, you can remove it with flame, but it's not a good idea to do so.
