Contact the HUD, Housing Urban Development, in your area.
I do know that you can get paper work for leases at places like Office Depot and Office Max, not sure about evictions.
You do not need a lawyer right now to serve the papers. Make sure the when you have the right paper work, send the eviction through mail, certified to your tenants. DO NOT give it to them personally. You can also have a company serve papers to them as well. It is in your best interest to have it mailed certified or have a separate company deliver to them so that it will be signed that they received it. Your tenants can come back and say they never got the eviction notice if you hand it to them or leave it on their door.
Once you have had the notice delivered, contact your attorney to let them know.

In most cases the state that you are in has web sites and assistance from the courts on how to go about evictions. They usually also have the forms that are approved by the state in the eviction process. In addition your state attorney general's office usually has pamphlets on what to do and not do in the event that you have to evict someone so you'll also have to research their site. At the court that you will file in. In my town we file at the Justice of the Peace. However, depending on the laws of your state, you will first have to give them a 3-30 day notice. Depending on the reason for eviction (non payment or otherwise). The court does not normally give you this.
