Depends on what kind of a change and condition and what my relationship with the client is

What do you mean by client?

If youre a hooker (because you could be, you havent clarified what you do) then Id stop sleeping with the client
If youre a hairdresser, Id comment on the condition of hair or scalp
If you are a dentist, Id ask if the clients been eating too many sweets
If you are a butcher, id say to the client theyve gotten fat on all the sausages theyve been eating

etc etc. VERY poorly worded question, more info is needed if we're to give a proper answer

depends if it is for the better or for the worse and it also depends on the condition they are being treated for in the first place

you need to offer more information. are you a health care worker? If so, do you mean as a registered practitioner, or a carer? Or are you in law?
