If you see nothing at all, then you have probably washed your specimen off the slide during the staining. I know because I have done this many times gram staining. Its more annoying when you are applying Ziel-Neilson staining.

Do you mean whether it's a systematic or random or human error? This would be considered a human error unless it's a limitation of your microscope which I doubt or else you would be using another one.

What stain? What kind of specimine?

Could be overstaining, could be understaining, could be no specimine...
