When you throw you fall off the mound plus you always recognize that the catcher is coming up firing. Also alot of the time you can see him out of the corner of your eye when you start your motion, sometimes. But its easy to see the difference in the catchers reaction to the ball when a guy is stealing and when a guy is not. Also, you know who's on base and you know their tendencies, so you kinda expect it.

1) your fielders always yell out that they r stealing, and 2) u can see the catcher come up fast and the pitcher ducks down

Communication, practice, and the plus for someone without great reaction time is the pitcher falls off to the side of mound when releasing the ball.

He reads the play from the reaction by the catcher and slips/ducks out of the way.

firstly the pitcher falls off to one side of the mound and secondly he usually hears the catcher or fielders say the guy is stealing

It's much easier to avoid than you think. The pitcher either ducks or steps to the side.
