if you put a new rotor on, yes you should also put fresh pads on as well.

Yes. The old pads will be grooved to fit the old rotors and probably have sand and grit embedded in them. You don't risk a new $100 rotor for lack of $15 pads.

Always change the pad when you change the girtle. HA! Yes, you want even wear on those new discs - your old pads shaped themselves to your old disc, so you wont get even braking til they wear down & shape to the new disc.

It is definately a good idea because you want everything to wear out at the same time so you can keep better track of their changing times. If wearing is uneven then it may cause damage to your discs.

if there are also new, or slightly used brake pads then no, but it is always a good idea to place new pads with a new rotor,.

no in general but is a good idea to do so, and, if you expend money on new discs expend a little more on pads
