How does cell division and enlargement lead to growth in plants?

Height of the stems and depth of the roots occurs at the tips of each. These regions are generally called apical meristem. Microscopically there are three distinct sections; Starting at the tips, and moving inward: Meristematic region (Mitotic/cell division area, but no real growth), Zone of elongation (growth area) and zone of maturation or differentiation (cellular specialization)
In the stems, growth in width (by way of cell division) occurs in the cambium. Seasonal growth in the cambium causes "rings" in some stems.
