You file a civil complaint. If you go to your state's web-site there should be a "how do I" section or a section about representing yourself in court. Occasionally they have forms.

Go to the courthouse and find out what the $ limits are so you ensure you file in the correct court. You need to file a Complaint stating why you are suing. You must have a copy served on the Defendant. Legal Aid will have sample petitions so you can file Pro Se.

If the Defendant fails to respond in 20 days, file a Motion for Default Judgment.
