It's been awhile since I had to diagram a sentence, but I'm pretty sure it is "Jane Addams" because that is the noun performing the action. "Hull House" is being acted on by the verb "opened" which makes it something else, that I can't remember the name of; and "Chicago" is not performing an action.
Someone back me up on this... I'm not all that sure.
