There's two different questions there.
Metaphysically, I believe their souls start to leave their bodies. My father often has a completely blank look on his face and doesn't respond to stimulus--it's like he's not in his head at all.
Physically, they can only remember the past because through some strange mechanism Alzheimer's destroys memories in reverse as well as randomly. They seem to be living in the past because the present has been erased. It's very scary for them and for their families.

My Grandmother has it - and lives in the past (when she isn't sleeping). She comes back once in a while and says she hates being the way she is. Alzheimer's is one of the cruelest diseases out there - for everyone in the family.....

I worked in a nursing home for 9 years. It was my experience that the dementia patience "settle" into a time of life that had a huge impact on them. It kinda like the movie "Ground Hog Day" for them. Unfortunatley, many of them are "stuck" in bad memories.

Most of the time it goes to their past.
Like they start talking about when they were little and the things that they use to do.

My grandpa has alzheimers. There mind doesnt go. They just lose their ability to recall anything.

Well my grandmother is going through that now, and she thinks she is back in PR where she was born. She keeps asking us for a penny to get some cookies.

In my grandfather's case, he could remember his name and things that happened in the past. But didn't seem to remember the present well.
