I worked at GameStop for 2 1/2 years as a Manager... the toothpaste thing DOES work, but it has to be PASTE, not the popular gel. Also, don't clean it with only alcohol. try balancing alcohol to water on a 1 -- 3 ratio. Also, try the Disk Doctor thingies. I've used them before to resurface disks and they're great. The only problem being you can only do it so much or you'll eat through the disk.
Also, be careful and read the directions on the resurfacers... I've seen a manual and electronic one. The manual one has a hand crank that if you turn the wrong way... you'll pretty much kill the disk. Also.. right before I left gameStop we got some things in that I've seen at Wal-Mart in a small metal disk-shaped tin called Disk Skins. It's a thin clear plastic cover that has a colored lip on it that you put on a disk to prevent or protect against scratches.
