i am residing in chennai. i have changed both my name and ressidence address. where should i apply to change in registered form. how should i change? where should i change? how long will it take? please suggest me.

I am afraid, it may not be possible for your to effect any change in the birth certificate, at this belated stage. What you may do is to get the changed name notified in the official Gazette, for a prescribed fees. On the basis of this Gazette Notification, you may have the name changed in all the official records, where you want the name to be changed.

Have the same problem, but start by looking up birth name and if you know the city and state, that is a plus go to deed of records at the court house there. Also if his name was changed then every thing should be under his changed name not unless he was adopted then had his name changed, that is a whole new can of worms

you have to make an affidavit in the court in the jurisdiction you are living,infront of a 1st class magistrate. then you have to obtain a form from the corporation where you have been staying .and apply alongwith the affidavit for change of name.,and addressand there should be a declaration in the newspaper.you can easily do it through an advocate within 15 to 20 days with alittle extra.

Changing address is not a big deal. A letter to the relevant authorities is enough. Changing name is the big deal.
You might have to consult a lawer for changing the name officially especially if you are doing business.

A notary can do it for you after publishing a notification in a news paper.
