Mustiness is from bacteria/mold and may be ingrained deep in the wood. Open up the chest and leave it open for a few months. If you want to kill the molds, you could take some bleach and use a spritz bottle all over the inside of the chest (dilute the bleach with water, maybe a few tablespoons of bleach + water in the spritzer). Spritz the entire chest, then respritz and let the bleach soak in. After using the bleach, let the chest dry out in the sun for a few days. Wait a week and sniff to see if the odor is gone (a slight bleach odor might remain). Any musty odor means spritz with more bleach solution. Keep repeating until the mustiness is gone. Then let the chest totally dry out. Buy some cedar oil and lightly brush the inside of the chest with the oil to restore the cedar odor. An alternative: if the chest's cedar panels are easily removed, take those out and replace them with new. It could be they can be easily replaced. Just depends on the way the chest was built. Next time you store the chest, make certain it is in a dry place, not a damp area. (I've dealt with a couple musty chests this way).

I would take it to a car wash. Not the ones you drive up and do yourself but the one with people. Sometimes they will do a wash of your inside fabric for extra cash.
If it is mold you are smelling that is bad and can make you sick. Unless it is visible you might have to pull up the carpet underneath. But it would only have mold if something was wet or left in it.
With all that said if it is just Must you have no problem. First vaccum the car, then go to Walmart (or something like it) and get an upholestry cleaner and clean the floors and seats. Make sure you do this on a nice sunny day so you can leave the doors open to let it dry out. After that spray febreze in it. That stuff works miracles (If your still worried about mold get the febreze anti bacteria one).

move to the desert and let it bake out. Roll down the windows on a dry heat day and let that mold and mildew bake to death, a hot dry wind is even better. If you're in San Diego, drive out to Palm Springs or Palm Desert--especially if the Santa Ana's are blowing Short of that if you can locate the source of the mildew or musty smell, you might try an extension cord from the house to a powerful hair dryer, and try to dry any spots that look like they may have been exposed to even slight moisture. Also spray Lysol is supposed to kill mold and mildew which often make that musty smell. You might try shampooing the carpets if you think moisture has gotten into it over the years. Hope that helps.

Clean it really well, air it out for as long as possible, Febreze the heck out of it, and then get an air freshener. It's also a good idea to use a fabric cleaner when you clean the car. If you want to do it the fast way, you could take it to a full-service car detailer. It's expensive, but if you have the money, it's totally worth it.

Get a spray bottle, put a tablespoon of white vinegar to a pint of water and mist the car, inside the vents also. Vinegar also kills mold, and bacteria. The other suggestions submitted are great to. You can also put carpet fresh in the ashtrays. If you get any on your windows, don't worry, it's a great window cleaner.

I know an excellent way, mix 70% milk and 30% white vinegar (do not use Malt vinegar) mixed with 1 tea spoon of salt, mix well, spray heavily on to the carpet and seats with a spray bottle, and allow the mixture to dry with the car closed in the sun, all your problems solved,

Put a couple of bags of opened charcoal in the car.
Some open cans of coffee are good too.
Sprinkle baking soda on the carpeting, then vacuum up.
