After 5 minutes, the plane would have travelled
(250)(5/60)
(250)(1/12)
20.83 miles

However, the 4 mile altitude must be taken into calculations as well.
Using the pythagorean theorem or its derivative the distance formula, you have:
d = Square Root of (20.83 ^ 2 + 4 ^ 2)
d = Sqaure Root of (433.89 + 16)
d = Sqaure Root of (449.89)

d = about 21.21

I'm still trying to figure it out, and I'll post the answer when/if I get it, but the key is SOHCAHTOA.
You have two sides of the triangle formed: The 4 miles between the radar station and the plane's height, and the 20.93 miles the plane has travelled since passing the radar station. You're now trying to find the hypotneuse. Give me a minute, and I'll have it for you.

thus x = 5 * 250/ 60 = 20.83

now base = 20.83
Ht = 4

the hyp = sqrt (ht^2 + base ^2)
= sqrt (16 + 433.889)
= 21.2105miles
