Everyone has their own approach, but here's how I did.
Start out learning a basic language (Java script). Sites like http://www.lissaexplains.com have great tutorials. Once you feel that you have mastered java script, buy a book for beginners for Java. I learned Java then took several classes in high school related to Java programming. Now I am in college level programming at a tech school. I find that if your planning on majoring in CS or even taking college level programming, you better know how to code before signing up.

completely and solely by myself.
I have never formally been taught programming. I learned by going on the internet, messing around, and by reading some books. Most of the time it was by trial and error.
However , I will be taking a computer programming course in high school next year in my sophomore year.

You learn your basics in high school and college. You get good at it by experimenting at home. Buy a junk computer and use it as a test station. That way, if you REALLY screw something up, you can always format your hard drive and start over.

Started when I was about 4 (no, I'm not kidding) with CBM BASIC. Moved up in the world to QBASIC around age 11 or so. PASCAL and Borland C around 13. HTML at around 18. Now I'm trying to get into C++ at 27. LOL
