No it is not legal. However as with everything else, there is not much you can do because if you call the labor board (that is who you would complain to) you will cause a situation where they don't want to work you and classify you as a hostile employee making your life hell even if they give in after the call from the Labor Board.
Now, if you don't have an account because you can't get one, you have a legitimate point to make to them. On the other hand, explaining why you can't get an account won't be easy. If you are having problems opening an account do a search for Chex Systems Sucks or something on the search engines to find a work around for a problem getting approved for an account.

No, an employer cannot force you to sign up for direct deposit, any more than he/she could tell you where you had to bank. It may be more convenient for you to do this though, since, in most cases, money will be immediately available to you, saving you the hassle of running to the bank. If you have been turned down by banks for opening accounts (for whatver reason), this would make it impossible to comply with the (illegal) demand anyway. Has the employer stated why they want direct deposit as the only choice for payment? Oftentimes it is because it is less costly for them, but they must give you a choice.

It all depends. Some places require you to sign up for direct deposit but i think the real question is why would you not want direct deposit. It is more convenient than getting a check, you don't have to worry about losing your check, and the money goes straight into your checking account when you expect it to be there with absolutely no problems. What else could you ask for unless you don't already have a bank account which i highly recommend you get anyway.

i'm not sure if its legal or not. but direct deposit is alot easier than taking a check or cash in hand. and where i live cash in hand is actually illegal in most cases.
if i were u i would just sign for the direct deopsit, or find another job that will pay you they way u want...

Absolutely. It is his money until he has incurred a contractual debt to you, his employee. It saves him/her a payroll staff, printing charges, and guarantees payment w/o interruption of the workday. If you don't have a bank....
