blood
face mask (pulled up over surgical hat) coat, rubber gloves, sack booties over your shoes. plus some comical pretend syringes, knives, rubber bands etc spattered with 'blood' and 'guts'
PS don't hang a stethoscope around yr neck because surgeons do not use them.
But you COULD paste an enormous grin on yr face, along with a few blood smears! - just for that extra effect.

One feature of doctors in costumes is they all carry the bag or
have an instrument. A masquerade show I went to for fashion models
recently offerred a new twist, candy like licorice red, and black or edible
stethescope you could make yourself. Then add thermos or bike bottle
that has a logo maybe with a sticker, dollar stores have great sticker ideas.

As a gag, you could get a bloody knife from the Halloween store - looks like it's full of blood.

Wear white high heels with it to give the costume a more feminine feel. If your also comfortable with this, I'd wear fishnets. (if its too risque for your situation, then don't). Maybe also a fake stethoscope around your neck?

maybe put like fake blood. and then like one of those mouth things that they wear. and plastic white gloves. and plain white close toed shoes, adn ur set
