I sing/scream and I get hoarse. Someone recommended grape juice.

practice using your diaphragm. Breath in, expanding your diaphragm (stomach area), and make a sound while you exhale. If you push in with your hands, you should get bursts of air in your sound. Speaking with your diaphragm instead of yelling with your throat takes away unnecessary strain.
And, yeah, you could try not screaming. Are you a teacher? ;-)

Boiled gingerroot will give your voice back when you lose it. Don't use it so often that you end up hurting your voice when you should be resting it, however. Slice about an inch or two of the gingerroot into thin slices and place in a pan of water on the stove. Bring to a boil and let it boil 10 minutes or so. Strain out the ginger pieces and drink the water. Sweeten it with a little honey if you need to in order to get it down.

The best way to keep your voice healthy is to use it properly. To figure out how to use your diaphragm in an upright position, sit in a straight chair. Loop your arms over the back of the chair and hold onto the chair as far down its back or legs as you can comfortably reach. It should stretch you just a little bit. Now slowly take a deep breath and fill your lungs up. Your shoulders should remain stationary and you should be breathing with your diaphragm. To train your self in using it, say the alphabet very slowly and with good projection and diction as you let the breathe out. Repeat this a few times morning and evening, and you should be on the way to keeping your voice healthy for many years to come. Use the gingerroot remedy as a last resort.

Learn to use it correctly. It will likely take training and practice, if this has become a habit.

Fresh lemon juice. Maybe mixed with grated fresh ginger and hot water added, with some honey, makes a lovely tea which will really help and soothe. But the fresh lemon juice is great in a pinch, just a couple teaspoons.

Drink plenty of water to keep your cells hydrated, don't smoke, and if you feel a strain on your vocal cords, stop sing/screaming. Also, practice a little each day until you work up to where you can do it for longer periods of time before straining.

Hot water with lemon and honey very soothing for the vocal chords and esophagus!
