There are not enough nuclear weapons that can be used "b4 we all die".
Likely scenarios are that North Korea would invade South Korea, perhaps bringing one along to punish their southern brothers for siding with the US and not coming under their (North) authority--they have been preaching for years that theirs (North) is the only Korean authority.
Another possibility is that they would claim to have a nuclear warhead on a missile and shoot it at Japan to punish Japan not only for historic wrongs but for breaking trade and causing them to lose face today. This won't work because their long range missiles are the only ones with the "lifting" capacity of a nuclear bomb and those are terribly unreliable. The reliable shorter range missiles can't heft a nuclear weapon, especially as crude as the North is likely to use, giving the Pakistani technologies. Pakistani missiles can deliver one to parts of India but North Koreans don't have Pakistani missiles.
Another scenario is that North Korea will indeed explode a bomb on the surface during a flurry of "deflecting" missile fire, then claim the US attacked them. This gives excuse to attack and conquer the south while they encourage allies China and Russia to attack the US with their nuclear weapons. Even if the ruse succeeds, the US will potentially lose between half to two-thirds of the population and Russia and China will no longer have large parts of their populations as well. The countries will still be there and still be functioning, but more at the level and example of Zimbabwe or Congo (Zaire) for a while. They don't all die, but the affected nations might certainly be hurting for a long while. But then this is the worse-case scenario.
Most likely, a permissive blow to North Koreans invading south will be made. South Korea will be horribly crippled. North Korea cautiously invaded (China and Russia will be using words like "proportionate response", ala Israel in Lebanon) and the North Koreans will crumble because of malnutrition and poor logistical supply (ala Soviet Union's disintigration).

Assuming you're an American, Romeo, the most encoraging thing to remember is that North Korea lacks the ballistic missile capability to get one of their nuclear weapons over here. See, the bomb is typically attached to the top of a rocket (similar to our old Apollo rockets). Some rockets contain many bombs or "warheads" that can land at different locations. North Korea has none of this technology, and can't get its bombs over here.
As other posters said... we have a lot of ballistic missles in the US (probably way more than we need), so that is also a deterrent for N Korea to strike us, even if they could get one of their bombs over here.

The US clearly have more nukes than everyone else, so North Korea would be mad to attack until they could match their firepower. Also, the explosion that the North Koreans set off the other day wasn't confirmed to be a nuke, just a large dirty bomb. Although devastating, a dirty bomb isn't _as_ devastating as a nuke.

North Korea has tested ONE nuclear bomb. Its output was 1 kiloton. That's 15 times less than Hiroshima. That's 6'500'000 times less than the world's nuclear arsenal (mostly in U.S. and Russian hands).
So let's put things in perspective a bit, shall we? ;-)
