Teddy Roosevelt is incorrect, he was made president after McKinley's assassignation in 1901 and re-elected in 1904. He left office in 1909.

FDR was elected to 4 terms in office, and is the only president to have ever served more than 2 terms. He was president from 1933 until 1945.

There is no president that was elected 3 times to office, even if you look at repeat elections - Cleveland is the only president to serve 2 non-consecutive terms.

Franklin Delano Roosevelt (January 30, 1882 – April 12, 1945), often referred to by his initials FDR, was the 32nd President of the United States. Elected to four terms in office, he served from 1933 to 1945, and is the only U.S. president to have served more than two terms.
Stephen Grover Cleveland (March 18, 1837 – June 24, 1908) was the 22nd (1885–1889) and 24th (1893–1897) President of the United States, and the only President to serve two non-consecutive terms.
Theodore Roosevelt, ran unsuccessfully for a third nonconsecutive term
