My testicles hurt if i dont get off... Like my girlfriend will play with them, then just stop and my stomache and testicles hurt/throb in pain for a bit.

When a man is sexually aroused, the semen builds up & the body prepares to ejaculate.
Stopping before orgasm but after a man has been stimulated & aroused causes the male body pressure. That is the cause for the pain and/or pressure you're experiencing.
You may want to avoid her playing with you, or...complete what you've begun & get your rocks off.

This is called "blue balls"... you get aroused to a point that you are so hard and excited that you gotta burst, but then you don't get to.... it all adds up and can REALLY hurt. I suggest strokin one off to get some good relief!
