You should look for any gynecologist, clinic or hospital, but make sure it's a yuu-sei-hogo-hou-shitei-i 優生保護法指定医, that is supposed to practice reliable and legal operations.
You can look for a match to these characters or, better yet, have a Japanese woman friend help you.
I'm not sure, but I have been told that it costs about half the fee you need when you're hospitalised for delivery.
On the Web, I found a person who said it costs US $800.00, and another who said it costs Yen 150,000 (about US $1,200.00).
