A skin is a customized user interface for a program or desktop that has a similar feel to the original but with a customized look. Most skins will come with a wallpaper, but they don't have to be combined.

Programs that allow you to "skin" basically allow you to graphically re-design the interface. Generally speaking you'll have to skin each program independently, if they offer the option. Each program will have different sites to download skins from, so it's hard to answer that part of the question - if in doubt, Google it! Skins aren't related to wallpaper or screensavers.
Something to bear in mind with a lot of skinning systems is that quite often skin-makers overdo the skins. By this I mean that they are quite often so OTT that it makes the program difficult to use, in some cases almost impossible. In some cases skins can be OTT in terms of the memory they take up, which will reduce the overall speed of your computer.
Pretty? Yes. Useful? Not so much. Necessary? Never.
