It's not so much the sound as the thought of fingernails peeling back from the fingers as they scrape across the board. For a true "painful" sound, watch the movie Psycho. Alfred hitchcock put a snake in a chimp pen and listened to them freak out (Warning others of danger, etc). He than took those sounds and transformed them to music which is why the shower scene in Psycho resonates so fiercly with Humans. It's the cry of ultimate danger and warning.

It's the sound's frequency level. The guys who figured it out earned an ig nobel prize for doing so! :-)
