I understand how you feel regarding this because I also have had friends who had this problem. The important thing to remember is that it is not your problem and not your business to fix. You can be embarrassed, disgusted, and alarmed all you want but you can do nothing to change this families' habits.
What you must remember is that you are her friend and if you do not like going to her house because it smells then just don't go there. If her parents are not the type of people who keep a clean house then your friend cannot do much to change their behavior. And anything you say will be even less likely to change this behavior.
If your question is should I say something to her then my answer is why would you. What do you expect to happen by telling her something that will embarrass her? Can she move out and get a place of her own? Probably not. So anything you say is only to let her know that you do not approve of her families' habits and how much it grosses you out. Your best way to help is to be kind to her and be a nice person.

Wow you could be talking about my childhood. My mother has an obsessive/compuslive disorder called hoarding. Our house was like a garbage dump. I couldn't have friends over and I was afraid to tell anyone. The best way to support her is to help her learn what a normal house is by having her over without expecting a a visit to her house in return. Also if your friend is lost about how to care for her own things www.flylady.net is a great and free support group that addresses dealing with a messy house productively. Your friend should establish her own area (like her room) to keep as she wants and not take her family's home personally. She should move out as soon as practicable and start her own household free of clutter.

Call that show, "How clean is your house". Or take a videotape or photo casually - and show her without mentioning the mess. Sometimes people need to see it in a different medium. Like your weight. Other than that, leave it alone, it's her parents house.

Clean the house with her.
Or clean it yourself and explain to her that she lives like a psycho and it's nasty. She could get an infection, sickness, or even a disease by living with animal feces.
If nothing sways her, might want to consider not being her friend or you yourself could get sick from her.

Tell her that her room is messy in a playful manner and help her clean it up. If you help her clean uo then you can chill with her in the cleanest quarter of the house.

always invite her over your house. Maybe you should hint around to always cleaning your room and your house. Say that one day you are sitting in her house and ask are they worried about all different kind of bacteria or germs line around in that mess.

Mind your own business. If you bring the subject up to her you will embarrass her beyond belief. If you don't like going over there then always suggest an alternative.

Obviously it isnt her fault, help her clean it up and maybe show her some articles on scabies and lice and tell her to share theinfo with her parents or call childrens services you can be anonymous and they will tell the parents to clean it up. No one should have to live that way.
